This report contains our assessment of the medical care provided to incarcerated people at California State Prison, Los Angeles County from August 2025 through February 9, 2026. We rated the case review component of the overall health care quality adequate and the compliance component of the overall health care quality adequate.

In this audit, the OIG assessed the department’s effectiveness in meeting the dietary needs of the incarcerated population and evaluated whether health and safety guidelines were being met. The department follows policies and procedures outlined in the Department Operations Manual (DOM), and the department’s Food Services Handbook.

This report contains our assessment of the medical care provided to incarcerated people at Wasco State Prison from September 2025 through March 8, 2026. We rated the case review component of the overall health care quality adequate and the compliance component of the overall health care quality adequate.

This report contains our assessment of the medical care provided to incarcerated people at Substance Abuse Treatment Facility and State Prison at Corcoran from July 2025 through January 5, 2026. We rated the case review component of the overall health care quality inadequate and the compliance component of the overall health care quality inadequate.

This report contains our assessment of the medical care provided to incarcerated people at Salinas Valley State Prison from June 2025 through December 7, 2025. We rated the case review component of the overall health care quality adequate and the compliance component of the overall health care quality adequate.

This report examines and analyzes trends in the healthcare provided across all California prisons during OIG’s seventh cycle of medical inspections, which took place between November 2022 and August 2025, with publication of the cycle’s final report in April 2026. The report also compares the inspection results of the same institutions across the three prior cycles – Cycles 4, 5, and 6.

This Special Review examines CDCR’s implementation of regulations governing placement of incarcerated people in restricted housing units within its prisons. We reviewed both historical California regulations on segregated housing confinement and regulations that became effective on November 1, 2023, that were intended to reform segregated housing confinement and implement restricted housing units. We assessed the department’s compliance with regulations and policies governing the placement and retention of incarcerated people in restricted housing, the living conditions within those units, and the effects of regulations and policies concerning violence against staff and the incarcerated population.

This report summarizes the California Department of Corrections and Rehabilitation’s performance in conducting internal investigations and handling employee discipline cases we monitored and closed from July 1, 2025, through December 31, 2025. This report combines two reports we have historically issued as separate reports and covers all staff misconduct investigations we monitored regardless of the manner in which the allegations were reported – allegations the department’s hiring authorities referred for investigation and allegations incarcerated people raised via the grievance and appeal process. Of the 226 administrative disciplinary and criminal cases we monitored and closed, we rated the department’s overall performance adequate in 110 cases, improvement needed in 48 cases, and inadequate in 68 cases. Of the 141 staff misconduct complaint cases we monitored and closed, we rated the department’s overall performance adequate in 44 cases, improvement needed in 41 cases, and inadequate in 56 cases.
