This Sentinel Case involves a male correctional administrator who allegedly made multiple derogatory and obscene comments to subordinate female staff and engaged in behaviors that violated the department’s Equal Employment Opportunity (EEO) and sexual harassment policies. The administrator also attempted to solicit information that concerned EEO complaints filed against him and intimidated a manager whom he supervised after the manager’s colleague had reported the administrator’s alleged misconduct. The hiring authority dismissed the administrator. However, without sufficient justification, the department subsequently entered into a settlement agreement with the administrator demoting him to a correctional officer position and suspending him for 11 months.

This report contains our assessment of the medical care provided to incarcerated people at California Rehabilitation Center between August 2022 and January 2023. We rated the case review component of the overall health care quality adequate and the compliance component of the overall health care quality inadequate.
This report summarizes the work we performed in 2023 reviewing 730 use-of-force incidents that occurred at California Department of Corrections and Rehabilitation (department) facilities. In this report, we present 14 monitored incidents in which we identified significant concerns stemming from issues that occurred prior to, during, and after the use-of-force incidents. We issued five recommendations to address concerns we identified while assessing the incidents and the various phases of the use-of-force review process.

In this audit we examined the accuracy and consistency of prison release date calculations for incarcerated people receiving determinate sentences and evaluated the department’s processes and procedures related to those calculations. We assessed the department’s process to ensure the accuracy of sentencing term release dates and to determine if the department properly applied time credits earned, forfeited, and restored in the calculation of sentencing term release dates in accordance with applicable laws, regulations, and guidelines.<br>
