This report describes the OIG’s monitoring of the department’s staff misconduct local inquiry process in 2024. From January 1, 2024, through December 31, 2024, the OIG monitored and closed 415 local inquiries. The OIG assigned one of three overall ratings for each case: superior, satisfactory, or poor. The department’s overall performance was poor in 270 of 415 cases, or 65 percent, and satisfactory in 145 cases, or 35 percent.

In this audit, we reviewed the department’s All-Hazards Emergency Operations Plan, as well as the 2024 site-specific emergency operations plans for 30 of the department’s prisons, assessing the department’s ability to evacuate prisons threatened by wildfires, floods, and earthquakes. We also completed an in-depth review of the preparedness of three prisons located in different parts of the state, reviewing their methods for assessing their individual risks for wildfires, floods, and earthquakes, their documented mitigation plans, and whether they revised and improved their policies in response to threats posed by natural disasters.

Staff in the Intake Processing Unit respond to complaints that the OIG receives through U.S. Postal mail, phone calls (toll-free hotline), and inquiries through our website, which can exceed 1,000 monthly claims. Below are 16 complaints that the Intake Processing Unit reviewed and closed as of February 2025. These cases highlight the OIG’s impact and efforts to resolve the complainant’s concerns.
